Parent Information: Please fill out the form and sign where indicated. In the case of an emergency, it is imperative that the school be able to reach the student's parent. This form will be used by the school staff when students are released to go home.

Nut-Free School Policy

Parent Information: RSS is “Nut-Free”School. RSS is committed to offering a safe educational environment for all students, including students with food allergies, by working cooperatively with parents, students, and staff to minimize the risks of accidental exposure to a food allergen while at school.

Please do not send any products containing peanuts or tree nuts with your child at school.The best way to determine whether or not the food contains peanuts or tree nuts is to carefully read both the ingredient labels to see if peanuts or tree nuts are a listed ingredient.

If your child has eaten peanuts or tree nuts before school, please be sure your child’s hands and face have been thoroughly washed with warm water and soap prior to coming to school.

In the event that any student brings an allergen containing a snack, the teacher will take/confiscate it, send it to the office and replace it with a “safe snack” provided by the school.

Medication Administration

Parent Information: Parents of students who require the administration of medication during the school day must have a PRESCRIPTION filled by the physician.

This form must be completely filled out annually and signed by the parent/guardian and the child’s health care provider before the child can be assisted with the administration of medication by the school nurse at the school site. The authorized health care provider must be licensed in California.

It is the parent/guardian’s responsibility to provide the school site with all necessary information and special instructions in writing related to the administration of medication to their child.

All medication must be picked up by a parent/guardian or adult designee at the end of the school year. NO medication will be given to a student to take home. Medication left in the school office at the end of the school year will be discarded.

Release of Information

The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student education records. Schools must have written permission from the parent or eligible student in order to release any information from a student's education record.

Duration: This authorization shall become effective immediately and shall remain in effect until one year from the date of signature.

Revocation: To revoke this request at any time before the disclosure, the parent or guardian must submit a written revocation. Copies of this signed authorization will be considered as valid as the original.

Visitation/Observation Policy

In order to ensure the confidentiality of all students in educational settings and the smooth operation of all classroom environments, the following standards of practice are required as part of the Rising Star Sped Academy observation policies.

Prior to observation, any observer, including the parent(s), shall complete the Classroom Observation Confidentiality Acknowledgement Form, which outlines standard confidentiality expectations for all adults who enter the educational setting.

If the observer is not the parent(s), prior to the parental representative observing a specific student at school, the parent(s) of the student shall provide written consent to Rising Star prior to the observation.
